In one my historical railroad books there is a re-print of an add that the copy was saying something like "we are adding a car between the locomotive and the first passenger car for passenger safety when the boiler explodes". Not IF, but WHEN. Not a good time to be the crew.

This was a boiler shell failure, a most atypical boiler explosion vs. a low-water crown sheet failure. I guess the explosion also severed the oco frame.
