Here is a specialized tool holder for getting in tight places and maintaining a stiff mount. The concept came from Model Engineer's Workshop. The tool holder is called a Piddington ?; Brits name all of their tools after the originator.
There is a second set of set screws underneath so it can be as a right or left handed holder.
You can make then up with as long a reach as needed, angled end, whatever features are needed.

Thanks. That's pretty nifty. The holders for my Norman-style NSQCTP (not-so-quick change tool post) allow the bit to be installed right at the edge of the holder, so that's half the battle. It would be easy enough to adapt the Piddington idea if need be.
